Patient Safety Program for Nursing Homes
During the next three years, CCME will be working with a select group of North and South Carolina nursing homes to focus their quality improvement efforts in key areas to improve the level of patient safety delivered to people with Medicare. The goal of this program is to achieve improved clinical health outcomes among residents while also promoting job satisfaction among staff in order to provide more patient-centered, consistent care.
Our nursing home quality and safety program includes:
- Preventing pressure ulcers.
- Lowering the usage of physical restraints.
- Providing quality improvement technical assistance to nursing homes in need.
- Performing clinical process assessments.
- Supplying educational services and resources on a variety of quality improvement principles and clinical quality measures.
- Identifying leadership roles and establishing clinical care teams.
- Sharing best practices to help facilities achieve a high level of quality care.
- Teaching a process for identifying and continuously improving quality of care for nursing home residents in the Carolinas.
