Physician Offices

CCME offers physician offices a variety of quality improvement services. We have partnered with CMS to provide an electronic health record (EHR)-based care management program called EHR Preventive Care Program to participating North and South Carolina primary care physician practices. The purpose of the program is to improve patient care by fully utilizing care management functionality within the EHR. This program will focus on improving care on preventive measures through:

• Breast Cancer Screening
• Colon Cancer Screening
• Flu Immunizations
• Pneumonia Immunizations

Our program offers participating physician practices the opportunity to work with professional consulting staff to implement EHR-based care management features that will improve patient care, help with workflow and process redesign, and provide assistance with electronic data reporting.

Our program also offers the following benefits:
