CCME: Regional Extension Center (REC) Learning and Action Network

The Regional Extension Center (REC) Learning and Action Network (LAN) will consist of REC-enrolled practices whose goal is to promote population health. All staff within the REC-assisted practice can participate. We will partner with The Office of the National Coordinator for Health Information Technology, the Health Information Technology Research Center, the REC, Beacon Communities, and Accountable Care Organizations.

Practice Assistance

  • Promote the engagement of the patient and family to improve patient health and self-management via secure and confidential communications (email or patient portals).
  • Promote Physician Quality Reporting System (PQRS), Patient Centered Medical Home, Meaningful Use, and other national initiatives.
  • Assist practices with benchmarking.
  • Assist practices using the care management functions of their electronic health record (EHR).
  • Assist practices with interpreting EHR data and reporting with attention to identifying and addressing disparities in care.
  • Provide educational programming, best practices, and intervention tools to improve use of preventive services and cardiac health.
  • Ensure practices utilize the EHR capabilities to establish QI programs.
  • Promote health information exchange.
